How to Apply

To apply for the 2013 Camper Sponsorship Program,

DOWNLOAD CAMPER SPONSORSHIP FORM HERE

✔ All fields are mandatory.
✔ You need to submit this form with a completed camp registration form clearly indicating which sessions you are attending (one form per camper).
✔ Your form must be endorsed by a member of your diabetes healthcare team and accompanied by proof of income (see details on application form).

The deadline for 2012-2013 requests is July 5th 2013. No late applications will be reviewed.
