How to Apply

Registration for York University's Diabetes Summer Sport Camps are administered by Diabetes Hope Foundation. Please see below for registration instructions:

1. Complete the registration form.

If you are applying for Camp Financial Assistance, please click here to download the form to see if you are eligible for financial support.

2. A parent or guardian must read and sign the Consent and Release Agreement, and Media Use and Release Agreement.

3. Make a Payment:
a) In person at Diabetes Hope Foundation by cheque*, cash, money order, Visa, Master-Card, or bank debit card during the following hours: Monday to Friday 9am to 5pm

b) Fax to 905-565-7296 (Credit card only; please include credit card number, expiry date and signature).

c) Mail to address below with cheque*, money order or credit card information. Do NOT send cash in the mail.

*Payment by cheque is accepted until June 4th,payable to York University and include the current date (post dated cheques will not be accepted).

Please use one registration form per child. Photocopies are acceptable. Registration will not be taken over the phone. Additional information will be mailed upon final placement of camper.

Financial Assistance
Through the generous support of the Alice and Murray Maitland Foundation, Bank of Montreal, LifeScan Canada, Medtronic of Canada, Shoppers Drug Mart and the Tracey Rubinoff Foundation, we have a limited number of subsidized camp spaces available for those who require financial assistance.
